Introduction (1-2 pages) – describe why this is an important issue or population

4. Proposed program (address each of the following areas)
a. Identify target population and its characteristics (at least 2 pages)
i. include specifics from the literature as well as from the local or intended community (2pages)
(this is usually more specific than in the literature review, here you go to the local level)

ii. What is your cultural and professional positionality? Describe your cultural socialization related to the population that is targeted by this program (choose 2 dimensions of identity that are relevant to
the population), e.g. ethnicity or race, social class, sexual orientation, age, disability, religion, gender, immigration status) (at least 1-2 pages)

b. Needs of the Population (2 pages) – this should be specific to the site that you are targeting for implementation
i. use literature, local community, interviews from community as well as potential stakeholders/consumers

c. Program objectives (Write measurable objectives) (2 pages)
i. what are the objectives of the program
ii. if there are specific components in the program, what are the objectives of those components (e.g.,
student learning objectives)

d. Determine how to deliver the services (3 pages) (this is the “how” – how will the objectives be met?
i. What is the method of delivery, what components make up the program (e.g., workshop,
electronic software, individual counseling, peer counseling, outreach, adventure travel, team
building activities, family meetings, etc.)
ii. Who will provide the services?

e. Determine the content of the program (4pages) (this is the “what” – what is the content of what will be
delivered? For example, what does the lesson plan consist of? What is the focus of the individual
counseling? What will the peer counseling cover or include?
